This is an application for Delivery of Mail Through Agent.
This form, along with 2 pieces of ID presented, only needs to be filled out once.
If there are multiple members utilizing an account, each individual will have to complete their own copy of the form.

This form must be submitted to us in person.  This can be done the first time you pick up.
